Get help with accessibility
Call 630-822-2242 or email care@paeonhealth.com to describe the difficulty you encountered and your preferred way to receive a response.
You can also visit our Contact page for office information and administrative assistance.
If an online task is difficult to complete, contact the office so we can discuss another way to help you access the information or service you need.
Report a website problem
When reporting a problem, please include:
- The page address or name of the online tool.
- What you were trying to do.
- What prevented you from completing the task.
- Your device, browser, or assistive technology, if known.
- Your preferred contact method.
You do not need to diagnose the technical issue. A brief description of your experience is helpful.
Please keep ordinary email and website messages limited to administrative information. Do not include medical records, diagnoses, insurance identification numbers, or other sensitive health information.
For website accessibility feedback, use our administrative contact form if you can access it. If the form is difficult to use, call or email the office instead.
Our accessibility goal
Our goal is to work toward the Web Content Accessibility Guidelines, WCAG 2.2 Level AA. These guidelines address areas such as readable content, keyboard navigation, text alternatives for images, and understandable forms.
We welcome feedback about barriers that make our website difficult to use. This goal describes our intended direction for improvement and is not a claim that every page or feature currently meets the standard.
Online booking and patient portal
Our website links to online appointment scheduling and Elation Patient Passport. These services have their own interfaces and features.
If you encounter difficulty using either service, contact our office and identify the step causing the problem. Please avoid sending screenshots that reveal private medical information through ordinary email.
Read our patient portal guide or first visit checklist for practice instructions. If you still encounter a barrier, contact our office.
Planning your office visit
For questions about building access or communication needs during an appointment, contact the office before your visit so we can discuss appropriate arrangements.
Paeon Health is located at 210 N Bolingbrook Dr, Suite A, Bolingbrook, IL 60440.

5 helpful tips for website accessibility
Tip 1: Make text easier to read.
Use your browser’s zoom control to enlarge the page until the text is comfortable to read. The control is usually in the browser menu. Your phone or computer may also offer larger text or screen magnification in its accessibility settings. If enlarged text overlaps or disappears, tell us which page you were viewing.
Tip 2: Try keyboard navigation.
If using a mouse is difficult, the Tab key moves between interactive items, such as links and form fields. Shift + Tab moves backward. Enter usually opens a selected link. If you cannot reach a control, see which item is selected, or continue through a form, contact us and describe where you became stuck.
Tip 3: Find information within a page.
Your browser’s Find command can help locate a word or phrase without reading the entire page. Search for terms such as “appointment,” “phone,” or “address.” On many computers, the shortcut is Control + F or Command + F. If you cannot locate the information you need, ask the office for help finding it.
Tip 4: Keep account information private.
When requesting help with online booking or the patient portal, describe the problem without sending your password, verification code, or medical records. You can explain which button or screen caused difficulty. If you share a screenshot, remove private information first. A description of an error message can often explain the issue without revealing account details.
Tip 5: Ask about another way to complete a task.
You do not need to keep repeating an online step that is inaccessible to you. Call the office and explain what you are trying to complete, such as finding practice information or requesting an appointment. Describe your preferred communication method and ask what alternatives are available for your accessibility needs.
