PRACTICAL VISIT GUIDANCE

Patient Resources: 6 Helpful Guides

Use these patient resources to prepare for appointments, manage routine requests, and find the right way to contact Paeon Health.

1. Prepare for Your First Visit

Complete requested intake forms before your appointment. Prepare a photo ID, insurance information if using insurance, and an updated medication list, including prescriptions, nonprescription products, vitamins, and supplements. Bring available medical records and a short list of questions, with your main concern first, to discuss with Dr. Sayyed.

Review our in-office visit information for your Bolingbrook appointment. For scheduling help or to confirm your visit type, call 630-822-2242. Our appointment FAQ explains new patient and follow-up visits.

2. Access the Patient Portal

Use the secure Elation Patient Portal for patient-specific messages and available results, records, forms, and documents. Keep your sign-in information private. If you need help accessing your account, call the office at 630-822-2242 for portal assistance.

Use the portal for questions about your health rather than the public website contact form or regular email. For medication, test, or referral questions, read our care and follow-up FAQ; further evaluation may be needed. For a medical emergency, call 911.

3. Review Insurance and Self-Pay Information

Our insurance patient resources explain what to check before scheduling. Confirm participation for your exact plan and network with your insurer and our office, and ask your insurer about your benefits and expected out-of-pocket costs.

If paying directly, review self-pay visit options and prices before booking. Ask which visit type fits your needs and whether tests, procedures, or other services may have separate charges. Use the office phone for general billing questions; keep insurance identification numbers out of the public website form.

4. Get Ready for Telehealth

Review these telehealth patient resources before your video appointment. Check your camera, microphone, and internet connection, and choose a private location. You must be physically in Illinois at the time of the visit. Review any appointment instructions provided by the office before your scheduled time.

Telehealth is available when clinically appropriate. Ask the office whether your requested visit can be handled by video, or check available appointments. If you need scheduling or connection assistance, call the office for instructions.

5. Arrange Medical Records

Recent laboratory results, specialist recommendations, and hospital discharge instructions may help with your visit. Ask the office which records are needed and how to send them securely. Bring available records when requested, or arrange a secure transfer from your previous practice according to the office instructions.

Follow the intake instructions provided by the office for requested patient forms. If you need a form or help arranging records, contact our Bolingbrook office for guidance. Keep medical documents and patient-specific details in the secure portal or the transfer method provided by the office.

6. Plan Your Follow-Up

Before your appointment ends, clarify your next steps, follow-up timing, and how you will receive results. Ask which instructions to follow and when to schedule another appointment. Keep a list of questions that arise afterward and use the secure patient portal for patient-specific communication.

Return to these patient resources when you need portal access, office contact information, or help preparing for another visit. Our primary care FAQ covers routine requests, including refills, tests, and referrals that may require further evaluation.
