How to book an appointment online
Choose the button for your visit type above. For a first adult office appointment, use Elation with New Patient preselected. For a new-patient video visit, use Zocdoc with New Patient Visit preselected. Established patients can use the separate video or office follow-up buttons. Follow the scheduling website’s steps to select a time and review the visit type, provider, and location before finishing.
Opening a scheduling link alone does not reserve a visit. Complete the requested steps and review any confirmation or instructions you receive. If you cannot find a suitable time or are unsure whether booking is complete, call the office for help.
What to prepare for your first visit
Review our first-visit checklist and complete any intake forms requested by the office. Have a photo ID, insurance information if using insurance, relevant medical records, and an updated medication list available. Include prescriptions, over-the-counter medicines, vitamins, and supplements.
Write down your questions, with your main concern first. Recent laboratory results, specialist recommendations, or hospital discharge instructions may help Dr. Sayyed understand your care. Ask the office which records are needed and how to send them securely before your appointment.
Getting ready for an Illinois video visit
Before your telehealth appointment, test your camera, microphone, and internet connection. Choose a private place where you can discuss your health, and keep your medication list and questions nearby. Follow any appointment instructions provided by the office.
You must be physically in Illinois during the video visit. Telehealth is available when clinically appropriate; some concerns need an office examination. If you need connection assistance or are unsure whether video fits your concern, call before the appointment. Our telehealth preparation guide has more information.
Check coverage before you book an appointment
Confirm participation for your exact insurance plan and network with your insurer and the office. Ask your insurer about benefits, referral requirements, and expected out-of-pocket costs. Our insurance information explains what to check; listing an insurer does not guarantee coverage for your particular plan.
If paying directly, review our self-pay pricing before booking. Ask which visit type fits your needs and whether tests, procedures, or other services may have separate charges. Call the office with general billing questions.
Office directions and appointment changes
For an office visit, check our office hours and directions before traveling to 210 N Bolingbrook Dr, Suite A, Bolingbrook, IL 60440. For a child’s appointment, call 630-822-2242 so the office can help with scheduling.
To ask about changing an appointment or a time you already selected, call the office. Have the appointment date, visit type, and scheduling website available so the team can help identify your booking. Follow any change or cancellation instructions included with your appointment.
Secure messages, records, and forms
Use the secure patient portal for patient-specific messages and available results, records, forms, and documents. Keep medical history, diagnosis details, and insurance identification numbers out of the public website contact form and regular email. For records from another practice, ask the office how to arrange a secure transfer.
For a medical emergency, call 911.
